These Terms of Service ("Terms") govern your access to and use of the Form Dash platform, operated by Form Dash LLC, an Iowa limited liability company ("Form Dash," "we," "us," or "our"). By creating an account or using our services at formdash.app, you agree to be bound by these Terms.
Form Dash provides an online platform that enables non-profit organizations, schools, sports teams, and similar groups ("Organizations") to create fundraising campaigns and forms, manage sales and donation events, and collect payments from participants and supporters. These Terms apply to administrators, coaches, staff, and any other individuals who create and manage an account on behalf of an Organization ("Administrators").

You are responsible for all activity that occurs under your account. Notify us immediately at help@formdash.org if you believe your account has been compromised.
Form Dash is designed for use by non-profit groups including K–12 schools, athletic teams, booster clubs, parent organizations, and similar community groups. You may use the platform to:
You may not use Form Dash for any unlawful purpose, personal commercial gain unrelated to your Organization, or activity that violates these Terms.
Form Dash facilitates payment collection on behalf of Organizations. All transactions are processed through our third-party payment processor(s). By using payment features, you agree to their applicable terms and conditions.
Form Dash may charge a platform fee per transaction or campaign. Current fee schedules are displayed within the platform and may be updated with notice. Continued use after a fee change constitutes acceptance of the new fee structure.
Refund eligibility is determined by the policies set by each Organization for their individual campaigns. Form Dash is not responsible for issuing refunds for funds already disbursed to an Organization. If you believe a transaction was made in error or without authorization, contact us promptly at help@formdash.org. We will work with the Organization and payment processor to investigate.
Collected funds, less applicable fees, will be disbursed to the Organization's designated bank account according to our payout schedule. Form Dash reserves the right to delay disbursement if we suspect fraud, a dispute, or a violation of these Terms.
Form Dash serves many Organizations that work with minors (individuals under 13). We do not knowingly collect personal information directly from children under 13. All account holders must be adults (18+) acting on behalf of an Organization.
If your campaign or form collects information from parents or guardians on behalf of minors (e.g., sports registration), it is your responsibility as the Administrator to:
Form Dash will not knowingly facilitate the collection of personal information from children under 13 beyond what is required to process a transaction authorized by a parent or guardian.
Your use of Form Dash is also governed by our Privacy Policy, available at formdash.app/privacy. In general:
As an Administrator, you represent and warrant that:
Form Dash and its underlying technology, trademarks, and content are owned by Form Dash LLC. You may not copy, reverse-engineer, or distribute any part of the platform without our express written permission.
You retain ownership of content you create on the platform (campaign descriptions, images, etc.). By uploading content, you grant Form Dash a limited license to display and use that content to operate the platform.
TO THE MAXIMUM EXTENT PERMITTED BY IOWA AND APPLICABLE FEDERAL LAW, FORM DASH LLC AND ITS OFFICERS, EMPLOYEES, AND AGENTS SHALL NOT BE LIABLE FOR:
Our total liability to you for any claim arising from these Terms or your use of Form Dash shall not exceed the greater of (a) the total fees paid by your Organization to Form Dash in the three (3) months preceding the claim, or (b) $100.
FORM DASH IS PROVIDED "AS IS" AND "AS AVAILABLE" WITHOUT WARRANTIES OF ANY KIND, EXPRESS OR IMPLIED. WE DO NOT WARRANT THAT THE PLATFORM WILL BE ERROR-FREE, UNINTERRUPTED, OR FREE OF HARMFUL COMPONENTS. YOUR USE OF THE PLATFORM IS AT YOUR OWN RISK.
You agree to indemnify and hold harmless Form Dash LLC, its officers, employees, and agents from any claims, liabilities, damages, and expenses (including reasonable attorney's fees) arising out of your use of the platform, your campaigns, your violation of these Terms, or your violation of any third-party rights.
We may suspend or terminate your account at any time if we believe you have violated these Terms, engaged in fraudulent activity, or pose a risk to other users or Form Dash. Upon termination, your right to use the platform ceases, and any pending disbursements may be held pending investigation.
You may close your account at any time by contacting us at help@formdash.org. Closing an account does not entitle you to a refund of fees already paid.
These Terms are governed by the laws of the State of Iowa, without regard to conflict-of-law principles. Any dispute arising under these Terms shall be resolved in the state or federal courts located in Black Hawk County, Iowa, and you consent to personal jurisdiction in those courts.
Before initiating any legal action, both parties agree to attempt in good faith to resolve any dispute through direct communication for a period of thirty (30) days.
We may update these Terms from time to time. We will notify Administrators via email or a notice in the platform. Continued use of Form Dash after the effective date of updated Terms constitutes your acceptance of the changes. If you disagree with any changes, you may terminate your account.
