FormDash is operated by Form Dash LLC, an Iowa limited liability company. FormDash provides an online event registration and fundraising platform for school-affiliated organizations, including dance teams and other student activity programs.

FormDash does not collect, store, or process credit card numbers. All payment card data is handled directly by Stripe, Inc., our payment processor. When you use FormDash, our hosting and infrastructure providers (Google Firebase) may automatically collect standard technical information including IP address, browser type, device type, and usage data. This information is used to operate and maintain the platform.
FormDash forms for events such as camps and clinics collect information about minor participants, including grade level, age, shirt size, and medical conditions or allergies. This information is submitted by a parent or legal guardian on behalf of the minor participant.
FormDash does not knowingly collect personal information directly from children under 13. All registration information for minor participants must be submitted by a parent or legal guardian.
We use attendee information only to fulfill the registration, provide it to the organizing team, and include it in the confirmation email sent to the registering parent or guardian. We do not use minor participant information for marketing or share it with third parties beyond what is necessary to operate the platform.

With Organizations: When you submit a registration or purchase through a FormDash-hosted form, your submission details (including attendee information) are made available to the organization that created the form. We retain organization account information for as long as the account is active and for a reasonable period afterward for financial recordkeeping purposes.
We retain buyer and attendee registration records for as long as reasonably necessary to fulfill the purposes described in this policy, including resolving disputes, processing refunds, and maintaining payout records.

Deletion: You may request deletion of your personal information by contacting us at help@formdash.org. We will fulfill deletion requests where we are not legally required to retain the information.
Email: Confirmation emails are sent as a necessary part of completing a transaction. These are not marketing emails and do not have an unsubscribe option.
FormDash-hosted forms are created by independent organizations. FormDash provides the technology platform but does not control the content of individual forms or the practices of the organizations that create them. We encourage you to review the policies of any organization whose form you complete.
Public-facing FormDash pages (such as payment forms and confirmation pages) may display advertisements served by Google AdSense, a third-party advertising service. Google and its partners use cookies and similar technologies to serve ads, which may be personalized based on your prior visits to this and other websites. Google's use of advertising cookies enables it and its partners to serve ads based on your browsing activity.

Ads are never shown on organization dashboards or account pages, and advertising partners do not receive your payment information or form responses.
