By registering for and or participating in this event, I acknowledge and agree to the following terms on behalf of myself and, if applicable, the minor participant listed in this registration.

Assumption of Risk
I understand that participation in events, activities, clinics, camps, performances, or fundraisers may involve physical activity and inherent risks, including but not limited to injury, illness, or property damage. I knowingly and voluntarily assume all risks associated with participation, whether known or unknown.

Release and Hold Harmless
To the fullest extent permitted by law, I hereby release, waive, discharge, and agree to hold harmless the event host(s), sponsoring organization(s), hosting facility, Form Dash LLC (the event registration platform), and their respective owners, directors, officers, employees, volunteers, agents, and affiliates from any and all claims, demands, actions, causes of action, damages, losses, costs, or expenses arising out of or related to participation in the event, including those caused by negligence, except where prohibited by law. This release applies to claims for personal injury, illness, death, property damage, or any other loss arising before, during, or after the event.

Medical Authorization
In the event of an emergency, I authorize event staff to obtain medical treatment for the participant if I cannot be reached. I understand that I am responsible for all medical expenses incurred and that the released parties assume no responsibility for medical care provided.

Participant Conduct
I acknowledge that participants are expected to follow event rules and instructions. The organizers reserve the right to remove any participant for unsafe, disruptive, or inappropriate behavior, with no refund obligation.

Refund and Cancellation Policy
Unless otherwise stated, event fees are nonrefundable. The event organizers reserve the right to modify schedules, activities, or cancel the event due to circumstances beyond their control.

Media Release
I grant permission for the event host(s) and their designees to photograph, video record, or otherwise capture the likeness, image, voice, or performance of the participant during the event. I authorize the use of these materials for promotional, marketing, educational, or fundraising purposes in any medium, without compensation or further approval. I waive any right to inspect or approve the finished materials.

Acknowledgment and Agreement
I certify that I am the parent or legal guardian of the minor participant, or an adult participant acting on my own behalf, and that I have read, understand, and voluntarily agree to all terms outlined above. I acknowledge that this agreement is binding and applies to all future participation related to this event.
